Botox for Chin Dimpling: What Causes an Orange-Peel Chin?

Relax your face.
Your chin looks completely normal.
Ready to smooth your chin? Learn more about Botox in Mesa, AZ at Allure Infinite Beauty.
Now close your mouth tightly, pout or move your lower lip.
Suddenly the skin across your chin develops little bumps and indentations that look almost like:
orange peel.
You may hear this called:
- Orange-peel chin
- Pebble chin
- Cobblestone chin
- Dimpled chin
And unlike many wrinkles we treat with Botox, this one often has less to do with aging than people assume.
A common contributor is a small but powerful muscle called the mentalis.
When that muscle contracts strongly, it can bunch the soft tissue of the chin and create the uneven, pebbled appearance you see in the mirror.
Botulinum toxin can sometimes temporarily relax an overactive mentalis muscle and smooth that movement-related dimpling. Reviews of mentalis treatment specifically identify botulinum toxin as a treatment for “cobblestone chin” associated with mentalis hyperactivity.
But—as we’ve said throughout this entire Botox series—
not every chin problem is a Botox problem.
Sometimes what looks like chin dimpling is only one part of a larger issue involving chin projection, facial structure, volume or the way the lower lip and chin work together.
Let’s figure out the difference.
What Is the Mentalis Muscle?
The mentalis is a paired muscle located in the center of the chin.
It plays an important role in movements involving the lower lip and chin.
When it contracts, it can:
- Elevate the soft tissue of the chin
- Help raise the lower lip
- Contribute to lip closure
- Create visible wrinkling or dimpling across the chin
The mentalis is the primary muscle targeted when botulinum toxin is used for a hyperactive or cobblestone chin.
This muscle is small.
But visually, it can have a big effect.
Why Does My Chin Look Like Orange Peel?
Try this.
Look at your chin while your entire lower face is relaxed.
Now press your lips together.
If the chin suddenly becomes covered in little dimples, the mentalis muscle may be contracting strongly underneath the skin.
Some people have relatively mild activity.
Others recruit the mentalis constantly.
You might notice it more when:
- Closing your mouth
- Pouting
- Speaking
- Concentrating
- Making certain facial expressions
Sometimes patients aren’t even aware they’re doing it until we show them.
That’s why we evaluate the chin while it’s moving, not just from a photograph.
Is an Orange-Peel Chin Caused by Aging?
Not necessarily.
You can have mentalis dimpling at a young age.
Muscle activity and facial anatomy play a major role.
But aging can make the appearance more noticeable because the tissues over the muscle may also change over time.
The lower face can experience:
- Collagen loss
- Skin laxity
- Volume changes
- Bone remodeling
- Changes in facial support
So a 25-year-old with a strong mentalis and a 55-year-old with mentalis activity plus lower-face aging may both say:
“I hate my chin.”
But they may not need the same treatment.
How Does Botox Help a Dimpled Chin?
Botulinum toxin temporarily reduces activity in the targeted muscle.
If excessive mentalis contraction is what causes the skin to bunch and dimple, reducing that contraction may create a smoother appearance.
Published literature describes botulinum toxin treatment of the mentalis for hyperactivity, chin dimpling and lower-face aesthetic concerns.
The important word is:
movement.
If your chin looks smooth when relaxed but dimples dramatically when the mentalis contracts, that tells us something.
The muscle is likely contributing significantly.
Is Chin Botox FDA Approved?
Using BOTOX Cosmetic specifically for chin dimpling or mentalis hyperactivity is an off-label cosmetic use.
Current BOTOX Cosmetic labeling includes adult treatment of moderate to severe:
- Glabellar frown lines
- Crow’s feet
- Forehead lines
- Platysma bands
Chin dimpling is not listed as a separate FDA-approved cosmetic indication.
Off-label treatment does not automatically mean inappropriate.
Botulinum toxin is used off-label throughout aesthetic medicine.
But the patient should understand:
- What is being treated
- Why it is being treated
- What result is realistic
- What alternatives exist
- What risks are involved
How Many Units of Botox Does a Chin Need?
We intentionally aren’t giving you a universal number.
There is no FDA-approved standardized BOTOX Cosmetic dosing regimen for orange-peel chin.
Published treatment approaches vary, and the mentalis deserves more respect than simply looking up a unit count online.
Why?
Because the mentalis participates in normal lower-lip movement and mouth closure.
Too much weakening, poor placement or unintended involvement of nearby muscles may create functional or aesthetic problems.
A review specifically focused on mentalis anatomy warns that inappropriate treatment can contribute to effects such as lower-lip changes, smile asymmetry and difficulty with mouth closure.
So once again:
start with anatomy—not units.
Why Is Treating the Chin More Complicated Than It Looks?
The lower face contains several relatively small muscles packed close together.
Around the mentalis are muscles involved in:
- Lower-lip movement
- Mouth-corner movement
- Facial expression
- Lip closure
That’s why a tiny area can still require careful treatment.
We aren’t simply trying to make the chin stop moving.
The goal is to reduce the movement creating the unwanted appearance while maintaining normal function and expression.
Could Chin Botox Change My Smile?
Potentially.
Anytime we’re modifying muscles involved in lower-face expression, this deserves discussion.
The mentalis works closely with the lower lip.
Nearby muscles also contribute to smile and mouth movement.
An anatomical review of mentalis botulinum toxin treatment specifically notes possible problems such as lower-lip ptosis and smile asymmetry when treatment affects surrounding function.
That doesn’t mean those things happen to everyone.
It means precision matters.
Tell your provider if your job, hobbies or lifestyle involve especially precise lip function.
And tell them if you’ve had previous lower-face Botox that changed your smile in a way you disliked.
What If My Chin Is Dimpled Even When I’m Relaxed?
Now we need to investigate further.
If the dimpling only appears during strong mentalis contraction, muscle activity is an obvious contributor.
But if the chin appears irregular even while relaxed, there may also be:
- Skin texture changes
- Scarring
- Structural irregularity
- Volume issues
- Previous filler
- Previous surgery
- Other soft-tissue factors
Botox may still help a muscular component.
But it may not completely eliminate an irregularity that isn’t being caused by muscle contraction.
Chin Botox vs Chin Filler
This is where things get especially interesting—and where we’ll transition into our filler series next.
Because chin Botox and chin filler can treat completely different problems.
Chin Botox
Primarily changes muscle activity.
Think:
- Dimpling
- Mentalis strain
- Cobblestone texture during contraction
Chin Filler
Primarily changes structure and volume.
Think:
- Chin projection
- Chin length
- Shape
- Certain contour deficiencies
- Facial balancing
Published literature on nonsurgical chin treatment discusses botulinum toxin for muscular activity and dermal fillers for structural concerns such as volume deficiency and underprojection.
Sometimes the patient’s concern involves both.
What If My Chin Looks Recessed?
This is an important distinction.
A chin can appear weak or recessed because of underlying skeletal structure and soft-tissue proportions.
Relaxing the mentalis does not physically build a larger chin.
However, research has shown that in some patients with a hyperactive, high-riding mentalis, reducing that muscular activity can alter the appearance of chin contour.
That does not mean Botox replaces filler for chin projection.
It means the muscle can sometimes influence how the underlying structure presents.
This is exactly why we want to evaluate both movement and structure.
How Can I Tell if I Need Botox or Filler in My Chin?
Ask yourself what bothers you.
“My chin gets bumpy when I close my mouth.”
That sounds muscular.
Let’s evaluate the mentalis.
“My chin looks too small from the side.”
That’s more of a structural conversation.
“My chin disappears into my neck.”
Now we’re evaluating projection, jawline, soft tissue and facial proportions.
“My chin is uneven even when I’m relaxed.”
We need to determine what is actually creating the asymmetry.
“My chin looks small AND it dimples.”
You may have more than one issue.
This is why asking:
“Do I need chin Botox?”
isn’t as useful as:
“What exactly is creating what I dislike about my chin?”
Can Chin Filler Fix the Orange-Peel Texture?
If the primary cause is strong muscle contraction, simply adding volume does not directly reduce that muscle activity.
Filler can change structural contour.
Botox changes muscular contraction.
Those are different jobs.
This is another example of why more filler isn’t automatically the answer anytime the lower face looks irregular.
And it’s exactly why our filler series is going to focus heavily on identifying volume problems versus muscle problems versus skin problems.
Can Botox Make My Chin Look Longer?
Sometimes relaxing a hyperactive mentalis can subtly change how the soft tissue of the chin sits.
A small case series evaluating onabotulinumtoxinA in patients with a hyperactive mentalis found measurable changes in vertical chin position and improvements in the perceived chin aesthetic.
But we would never promise:
“Botox will lengthen your chin.”
If true structural chin length or projection is the concern, that belongs in a facial-balancing discussion.
Why Does My Chin Wrinkle When I Close My Lips?
Sometimes patients are unconsciously using the mentalis to help maintain lip closure.
That can create visible strain across the chin.
This is particularly important to recognize because the mentalis has a functional role.
If someone is recruiting it heavily to close the lips, we want to understand why before simply weakening it.
Again:
The wrinkle is a clue.
It’s not always the whole diagnosis.
What Is Mentalis Strain?
Mentalis strain describes visible or excessive activation of the chin muscle.
You may notice:
- Chin dimpling
- A deep groove above the chin
- Lower-lip tension
- An upward bunching of the chin
- Constant contraction while the mouth is closed
The appearance can sometimes become much more obvious during facial movement.
Modern reviews of off-label aesthetic botulinum toxin treatment include mentalis strain among commonly addressed lower-face concerns.
Can Botox Improve the Horizontal Line Above My Chin?
Maybe—but first we need to determine what causes the line.
The crease between the lower lip and chin is called the labiomental crease.
Its appearance can be influenced by:
- Bone structure
- Chin projection
- Lower-lip anatomy
- Mentalis activity
- Soft-tissue volume
- Aging
Botox may affect a muscular component.
But a deep structural crease isn’t necessarily something we should chase with increasing amounts of neuromodulator.
This is where evaluating the chin as part of the whole lower face matters.
What About a Downturned Mouth and Chin Dimpling Together?
These concerns sometimes appear together because several lower-face muscles interact.
A downturned mouth may involve activity of the depressor anguli oris, commonly called the DAO.
Chin dimpling involves the mentalis.
These are different muscles.
So if someone says:
“Can you Botox my lower face?”
we need more information.
Are we treating:
- Chin dimpling?
- Downturned corners?
- Lip position?
- Facial asymmetry?
- Several different concerns?
Each muscle needs a reason for being treated.
Can Botox Sharpen My Jawline if I Put It in My Chin?
Not really in the way people often mean.
Chin shape contributes to the overall appearance of the lower face.
But mentalis Botox isn’t a jawline-contouring treatment by itself.
If your primary goal is:
“I want a sharper jawline.”
we need to evaluate:
- Chin projection
- Jaw shape
- Submental fullness
- Skin laxity
- Platysma activity
- Masseter size
- Facial volume
- Overall proportions
The lower face is a system.
One injection point doesn’t solve every part of it.
Can Chin Botox Help a Double Chin?
No.
A double chin usually refers to fullness beneath the chin.
That can involve:
- Submental fat
- Skin laxity
- Anatomy
- Neck structure
Relaxing the mentalis doesn’t remove fat.
It doesn’t tighten loose neck skin.
It doesn’t remodel the jaw.
That’s a completely different treatment conversation.
Does Chin Botox Work Immediately?
No.
Botulinum toxin requires time to affect muscle activity.
Because orange-peel chin treatment is an off-label use, there isn’t a specific FDA-approved timeline for this indication.
Your provider should tell you when they want you to evaluate the result.
Don’t judge the treatment five minutes after leaving the room.
And don’t immediately add more simply because movement remains early on.
How Long Does Chin Botox Last?
The effect is temporary.
Exactly how long an off-label mentalis treatment lasts varies by patient, product and treatment.
Published clinical observations show the muscular effects and aesthetic improvements diminish as botulinum toxin activity wears off.
If you like the result, repeat treatment may eventually be needed to maintain it.
But we don’t believe treatment should happen automatically because the calendar says it’s time.
Evaluate the movement.
Evaluate the concern.
Then decide.
Can I Become Dependent on Chin Botox?
Not in the sense that your chin becomes incapable of existing without it.
As the neuromodulator effect diminishes, muscle activity returns toward baseline.
You may simply become more aware of the original dimpling because you’ve spent several months seeing the area smoother.
Cosmetic treatment is optional.
You can continue.
You can stop.
Your chin doesn’t sign a lifetime Botox contract.
What If Chin Botox Makes a Weird Bulge?
Interestingly, this has been reported.
A published case report described paradoxical mentalis bulging after botulinum toxin treatment, thought to be related to uneven relaxation of portions of the muscle.
That’s another good example of why this seemingly tiny treatment isn’t just:
“Put Botox in the middle of the chin.”
The muscle has anatomy.
The treatment needs strategy.
If anything looks unexpectedly asymmetric or abnormal following treatment, contact your treating medical provider rather than trying to diagnose or correct it yourself.
Is Chin Botox Safe?
Botulinum toxin treatment of the mentalis has been described extensively in aesthetic literature, but this remains an off-label use when we’re specifically talking about BOTOX Cosmetic for chin dimpling.
BOTOX Cosmetic itself is a prescription medication with contraindications and warnings.
Its current prescribing information includes a boxed warning regarding possible distant spread of toxin effect and emphasizes the importance of understanding the relevant neuromuscular and structural anatomy.
In the chin specifically, poor treatment selection or placement can potentially affect lower-lip function or smile symmetry.
We don’t say that to make a small treatment sound terrifying.
We say it because:
“Small treatment” does not mean “zero-risk treatment.”
Who Might Be a Good Candidate for Chin Botox?
The conversation may make sense for someone who:
- Has visible orange-peel or cobblestone dimpling during mentalis contraction
- Has noticeable mentalis strain
- Understands that the treatment is temporary
- Understands that the use is off-label
- Has realistic expectations
- Has been appropriately medically evaluated
But candidacy can’t be determined from a blog.
Who Might Need Something Other Than Botox?
If your primary concern is:
A weak or recessed chin
Think structural evaluation.
Poor chin projection
Filler or another structural treatment may be more relevant.
Loose skin
That’s not primarily a mentalis problem.
A double chin
That’s a different anatomical issue.
A deep crease that’s present without movement
We need to determine whether structure, volume, skin or previous treatment is contributing.
Facial imbalance
The entire profile may need to be evaluated rather than treating one muscle.
The best treatment may ultimately be filler, Botox, another modality, a combination—or nothing at all.
Why We Don’t Want to Treat a Chin From a Selfie
A photograph can show shape.
But it doesn’t always show function.
During a chin consultation, we may want to see you:
Relax.
Close your mouth naturally.
Press your lips together.
Pout.
Smile.
Talk.
Then we may look at your profile.
We’re evaluating two different things:
What does the chin look like?
and
What does the chin DO?
That distinction can completely change the treatment plan.
Botox for Chin Dimpling vs Facial Balancing
This is going to become especially important as we move into our filler articles.
Maybe someone hates their chin because it’s bumpy.
That’s a mentalis conversation.
But maybe the bigger issue is:
- A recessed profile
- Limited projection
- An imbalance between the nose, lips and chin
- Poor lower-face definition
Now we’re talking about facial balance.
Sometimes fixing the thing a patient first notices isn’t the same thing as addressing the thing that makes the biggest aesthetic difference.
That’s where consultation becomes valuable.
Our Approach to Chin Botox at Allure Infinite Beauty
If you point at your chin and say:
“I need Botox here.”
We’re probably going to ask:
“What bothers you about it?”
Then we’re going to watch it move.
Is the skin smooth while relaxed?
Does the chin immediately pebble when you close your lips?
Is there significant mentalis strain?
Does the chin look structurally recessed?
Is one side different?
Are you trying to fix texture—or are you actually unhappy with the entire shape of your lower face?
Those are different problems.
If strong mentalis activity is creating the concern, neuromodulator treatment may make perfect sense.
If the problem is primarily structural, Botox may accomplish very little.
And if it’s both?
Then we discuss both.
We aren’t trying to turn every chin into a Botox treatment.
We’re trying to figure out why you don’t like what you’re seeing.
Then we choose the appropriate tool.
Frequently Asked Questions About Botox for Chin Dimpling
What causes an orange-peel chin?
A common cause is strong contraction of the mentalis muscle, which can bunch the overlying skin and create a pebbled or cobblestone appearance.
Can Botox smooth a dimpled chin?
Botulinum toxin can temporarily reduce excessive mentalis contraction and has been used to improve cobblestone chin and mentalis strain.
Is Botox for chin dimpling FDA approved?
Chin dimpling is not among the current FDA-approved BOTOX Cosmetic indications. It is an off-label cosmetic use. Current approved aesthetic indications include frown lines, crow’s feet, forehead lines and platysma bands.
How many Botox units are needed for an orange-peel chin?
There is no FDA-approved universal chin-dimpling dose. Treatment should be individualized based on anatomy, muscle activity, product and the provider’s clinical judgment.
Can Botox in the chin change my smile?
Potentially. The mentalis is close to muscles involved in lower-lip movement, and inappropriate treatment can contribute to effects such as lower-lip changes or smile asymmetry.
Can Botox make the chin look longer?
Relaxing a hyperactive mentalis may subtly affect chin contour in selected patients, but Botox does not physically build or significantly augment the chin.
Is chin Botox the same as chin filler?
No. Botox changes muscle activity. Filler changes volume and structure.
Which is better: chin Botox or chin filler?
It depends on the concern. Dimpling from mentalis contraction may respond to botulinum toxin, while projection or structural deficiency may be better addressed with filler or another structural treatment.
Can I get chin Botox and chin filler together?
Some patients may have both muscular and structural concerns, so combination treatment may sometimes be considered. Each treatment should have a clearly defined purpose.
Will chin dimpling come back after Botox?
Because botulinum toxin effects are temporary, mentalis activity and the associated dimpling can gradually return as the treatment wears off.
Is a dimpled chin normal?
Yes. Mentalis contraction and chin texture vary between people. Treatment is cosmetic and only worth considering if the appearance bothers you.
Can Botox fix a recessed chin?
Botox does not add structural projection. A recessed chin requires a broader evaluation of skeletal anatomy, soft tissue and facial proportions.
